Populations Most at Risk of HIV Infection Have Worst Access to PrEP: STUDY
A new study from the University of California has found that those most at risk of contracting HIV have the least access to pre-exposure prophylaxis (PrEP).
Populations that are most vulnerable to seroconversion are Latino and Black men who have sex with men (MSM), transgender women, and those who live in southern states.
In a survey of gay and bisexual men in California, only a handful of participants reported having taken PrEP. PrEP use was highest among young white men, at 13.9 percent. For young Latino men, that figure was cut by more than half, while young black men represented less than 10 percent of people who started PrEP.
“This is not reflective of the HIV epidemic at all,” says Shannon Weber, founder of Please PrEP Me, an online directory of over 230 clinics in California that provide PrEP. “It is reflective about access, and where and how people are getting that information.” […]
“Black and Latino men who have sex with men, who are overrepresented in the epidemic, have low threshold access to the services,” says Dr. Demetre Daskalakis, assistant health commissioner overseeing the Bureau of HIV/AIDS Prevention and Control at the New York City Department of Health and Mental Hygiene.
It’s a broad racial disparity first reported in June by Gilead Sciences, the biopharmaceutical company that makes Truvada. White gay and bisexual men made up the biggest gains in PrEP usage in 2015, although they’re far less likely to contract HIV in their lifetime. PrEP use among black men, on the other hand, has dipped since 2012. So while prescriptions for Truvada continue to rise significantly, McCord says, “What we’re not seeing is perhaps where PrEP is needed and that is in higher risk communities.”
Interestingly, Republican opposition to Obamacare plays a factor in the limited availability of PrEP in the American south:
Those Southern cities most susceptible to an HIV outbreak – El Paso, Atlanta, Jackson, to name a few – are also in states that haven’t expanded Medicaid, which does cover the costs of Truvada after a small co-pay. That means the preventive drug, which can cost upward of $1,564 for a month’s supply, has become financially out-of-reach for hundreds of thousands of eligible PrEP users because GOP lawmakers refused to make healthcare accessible to people living in poverty. “Low cost services for HIV-negative people [are] practically nonexistent,” says David Evans, director of Research Advocacy at Project Inform.
Some major population centers have, however, begun expanding access to PrEP for at-risk communities:
Last year, San Francisco and Los Angeles ramped up efforts to increase PrEP use among men who sleep with men and transgender women after receiving grants from the CDC as part of the agency’s Project PrIDE initiative. In June of this year, MAC AIDS Fund announced a two-year, $1 million citywide program in Washington, D.C. to educate and promote PrEP use among black women — one of the groups most at risk in a city that once had the worst HIV rates in the country. Last week, researchers from the Rollins School of Public Health at Emory University in Atlanta launched a searchable nationwide PrEP provider database. And in the coming months, about 70 community-based clinics in underserved areas throughout New York City will start providing PrEP with the support of the city’s Department of Health and Mental Hygiene.
“People who don’t have a voice aren’t going to be walking into a clinic with a giant rainbow flag on it,” like the ones in Chelsea, the West Village and other LGBTQ-friendly neighborhoods, says Daskalakis. “We’re creating the infrastructure in places that may not have that giant rainbow flag, and we’re really proud of that.”

Scotland Moves to Pardon Gay Men Convicted Under Outdated Indecency Laws
Scotland plans a blanket pardon for gay men convicted under outdated indecency laws, the Guardian reports:
Holyrood’s justice secretary, Michael Matheson, told the chamber on Tuesday afternoon that the parliament would bring forward a Scottish “Turing law” to automatically pardon gay and bisexual men convicted of sexual offences that are no longer criminal.
He added that his officials had also been in discussion with Police Scotland to identify the most effective way of ensuring that past convictions for consensual sex between men no longer appeared on a person’s criminal record.
A similar law intended to apply to England and Wales was scuppered by the Conservative justice minister, Sam Gyimah, on Friday after he spoke so long that it ran out of time.
The BBC reported earlier this week that Gyimah had proposed an alternative bill that “would grant similar pardons for those who have died but would make those still alive go through a ‘disregard process’ to receive official pardons.”
The Scottish MPs hope to avoid that application process and offer a full blanket pardon.

Labour MP Chris Bryant got emotional when talking about the new law, which has been proposed as the Turing Law, after Alan Turing.
Bryant wants the bill named after Ronald Cartland, Jack Macnamara, Anthony Muirhead, Robert Bernays and Victor Cazalet, a group of gay MPs from the 1930s.

730 More North Carolina Jobs Lost Under Governor McCrory’s Watch
HRC President Chad Griffin and Equality North Carolina Executive Director Chris Sgro released the following statements after news broke today that North Carolina lost out on 730 jobs because Pat McCrory has refused to support the full repeal of HB2.
“With each passing day that Governor McCrory’s discriminatory HB2 remains law the people of North Carolina continue to pay the price,” said HRC President Chad Griffin. “North Carolina and its people are suffering under HB2, and yet Governor McCrory and his allies continue to defend the indefensible. Governor McCrory has proven he is unfit to lead this great state, and the majority of fair-minded North Carolinians will finally have a chance to make their voices heard on Election Day.”
“What a travesty — our state has lost 730 more jobs because of Governor McCrory’s bad bill,” said Equality NC Executive Director Chris Sgro. “HB2 puts LGBT people at risk for discrimination and violence and it’s hurting all North Carolinians every day. Where are Pat McCrory’s priorities? 200 days after he signed this bill, he still refuses to step up and fix the mess he made of our state. Shame on him for putting his politics above our economy.”
BACKGROUND:
Pat McCrory has refused to fully repeal HB2, despite massive economic fallout as companies concerned with protecting their consumers and employees have moved conventions, trainings, operations and productions out of state. Since Gov. McCrory and state lawmakers rammed HB2 into law, the outcry has continued to grow:
- The NBA announced it was moving the 2017 All-Star Game because of the failure of the General Assembly to repeal HB2, costing North Carolina an estimated $100 million in All-Star Game related profits.
- The NCAA, the ACC and the CIAA also announced they would stand up for the safety of their employees, players, and fans by moving championship games out of North Carolina due to the state’s refusal to repeal its anti-LGBTQ HB2 law.
- More than 200 major CEOs and business leaders signed an open letter calling for full repeal of HB2 – including many of North Carolina’s largest employers. More than 50 investment managers with more than $2.1 trillion in investments signed a similar letter.
- Major film studios and corporations, from PayPal to Deutsche Bank, have stopped investments in the state because of the new law’s threat to employees and consumers. Conventions have withdrawn from the state, taking substantial revenue with them. Prior to the NBA and NCAA decisions, the Tar Heel State had already taken a hit of at least $329.9 million in lost business, and in taxpayer money used to defend the measure – including funding Gov. Pat McCrory’s road trips to explain why he signed discrimination into law.
- Artists including Beyoncé, Bruce Springsteen, Dead & Company and Cyndi Lauper have spoken out.
- In May, the U.S. Department of Justice (DOJ) filed suit in federal court, stating that HB2’s state-mandated discrimination against transgender people, including government workers and students, violates Title VII of the Civil Rights Act of 1965, Title IX of the Education Amendments of 1972 and the Violence Against Women Act of 2011.
- Joined by 68 major companies, HRC filed an amicus brief in support of DOJ’s effort to block some of the most egregious and discriminatory components of HB2.
- Duke University men’s basketball coach Mike Krzyzewski called the bill “embarrassing” and North Carolina State University men’s basketball coach Mark Gottfried said it “appalled” and “embarrassed” him.
