Good, Clean, Gay Sex: Best Practices for Better Bottoming<\/b>

Body Conscious: Gay Men\u2019s Health, Sex, and Self<\/a> is a new regular twice-monthly feature from Towleroad covering the unique issues gay<\/a> men<\/a> face with regard to physical and mental fitness.\u00a0<\/em><\/p>\n

The results of a new survey of 300 gay<\/a> men<\/a> were recently released by Bespoke Surgical<\/a>, a self-described \u201celite anal surgery practice\u201d focused on sexual wellness. The company focuses on preventative screenings, anal rejuvenation including botox<\/a> and bleaching as well as anal restoration services like skin tags, anal warts, hemorrhoids, abscesses, fistulas, and fissures.<\/p>\n

\"BodySo it\u2019d make sense they would want to know more about how gay<\/a> men<\/a> feel about anal sex<\/a> and their preparation for it.<\/p>\n

One question posed by the company focused on condom usage. They found that 37% of their respondents always use a condom during anal sex<\/a>, 21% occasionally do, and 16% never use a condom.<\/p>\n

The company found that among its respondents hygiene was a major concern before and after anal sex<\/a>.<\/p>\n

78%<\/strong> of those polled said they think about cleanliness always or most of the time during anal sex<\/a>, while just 9%<\/strong> never think about it. And clean-up was important to 61%<\/strong>, who said they would feel more comfortable if they used some sort of anal cleanser afterwards.<\/p>\n

As for the frequency of anal sex<\/a>:<\/p>\n

39%<\/strong> \u2013 a couple<\/a> times a week.
24%<\/strong> \u2013 a couple<\/a> times a month.
13%<\/strong> \u2013 daily<\/p>\n

Writes Bespoke: \u201cPut another way, 5 in 6 gay<\/a> men<\/a> have anal sex<\/a> at least a few times each year. Only 1 in 6 partake less than once each year. This information has implications: it confirms that the topic of anal sex<\/a> preparation is something that impacts many people. Plus, if it\u2019s something that most people don\u2019t do properly, it\u2019s a issue about which people need to be educated.\u201d<\/p>\n

As far as preparation goes, those polled by Bespoke on their use of a douche before sex<\/a> replied with these answers:<\/p>\n

42%<\/strong> \u2013 always or frequently use an anal cleanser
38%<\/strong> \u2013 rarely or never
21%<\/strong> \u2013 sometimes<\/p>\n

They add: \u201cInterestingly, gay<\/a> men<\/a> who have anal sex<\/a> daily are 1.3 \u2013 2.1 times more likely to always use an anal cleansing product, compared to those who do it less often. We found that, generally speaking, the less frequently someone has anal sex<\/a>, the more likely they are to never use an anal cleansing product.\u201d<\/p>\n

Bespoke also found that, among its respondents, 25%<\/strong> used a water-based enema to prepare for sex<\/a>, 20%<\/strong> used a saline enema, and 18%<\/strong> used a shower adapter.<\/p>\n

\"\"But should you even be douching at all?\u00a0Not according to some experts.<\/p>\n

Dr. Evan Goldstein, who founded Bespoke, told VICE that douching too often can lead to other health issues<\/a>: \u201cIf you\u2019re doing it every day, there are a lot of issues with enemas. You can lengthen your colon, and that can lead to constipation later in life.\u00a0That\u2019s what\u2019s so dangerous about it.\u00a0It\u2019s not threatening, but it\u2019s a real pain in the ass<\/a>, so to speak, it can make you uncomfortable and miserable.\u201d<\/p>\n

Goldstein said that there\u2019s little chance you\u2019re gonna run into anything up there anyway: \u201cImagine if you line up ten guys<\/a>, and you\u2019re going to top them all.\u00a0Even if they haven\u2019t prepared, nine out of ten would be completely stool-free.\u201d<\/p>\n

And Dr. Stephen Goldstone,\u00a0an assistant clinical professor of surgery at Mount Sinai Hospital and a specialist in anorectal disease and gay<\/a> men<\/a>\u2019s health, agreed, explaining that feces stay in an upper part of the colon called the sigmoid colon until you\u2019re ready to take a dump, so encountering them during sex<\/a> isn\u2019t going to occur most of the time.<\/p>\n

Also, don\u2019t use Fleet or other based saline enemas because the chemicals in them are super harsh on your rectum, causing irritation, along with dryness and mucus issues that can lead to bleeding and make transmission of STDs more likely.<\/p>\n

If you want to make a change, try adding more dietary fiber to your diet, and consider just washing your anal area before sex<\/a>. If you absolutely must douche, do it with plain water, and not too much of it.<\/p>\n

Bespoke\u2019s survey also asked about sex<\/a> toys, and anal damage. They reported: \u201cWe found that 41%<\/strong> of gay<\/a> men<\/a> have experienced tearing or other damage from anal sex<\/a>. Finally, the data tells us that 45%<\/strong> of gay<\/a> men<\/a> don\u2019t use a sex<\/a> toy for anal sex<\/a> at all, while 41%<\/strong> use one at least to prepare. 55%<\/strong> use one for pleasure, to prepare, or both. It\u2019s possible that those who use a toy to prepare may be doing so to avoid tearing or other damage.\u201d<\/p>\n

Bespoke\u2019s survey found that 39%<\/strong> of men<\/a> said they were tops, 29%<\/strong> identified as bottoms, and 33%<\/strong> described themselves as versatile.<\/p>\n
